This page explains how this website handles the information you provide through it. It is separate from, and narrower than, the confidentiality that governs actual therapy sessions and clinical records, which is described in a separate notice you receive at intake.
The Contact Form
What the Contact Form Collects
The contact form on this site asks for your name, an email address, and whatever message you choose to write. That information is used for one purpose, so Athena Metos can reply to you by email about scheduling and beginning therapy. It is not sold, rented, or used for advertising, and it is not added to a marketing list.
The form is a simple web submission, not a secure clinical portal. Please do not use it to send protected health information: symptoms, a diagnosis, medication names, or insurance identification numbers. A short message describing that you are looking for support is enough; anything more clinical in nature belongs in the intake process once you are scheduled, through a channel appropriate for that kind of detail.
Submitting the contact form, or otherwise browsing this website, does not create a therapist-client relationship. That relationship begins only once an appointment has been scheduled and the appropriate consent and intake paperwork has been completed.
Clinical Records
Therapy Records Are Governed Separately
Once you become a client, the records created in the course of therapy (session notes, intake forms, treatment history) are governed by the practice's clinical confidentiality obligations and by the HIPAA Notice of Privacy Practices provided to you at intake, not by this website policy. That notice describes in detail how clinical records are used, stored, and protected, and the specific circumstances in which disclosure may be legally required.
